Unfortunately, we are not accepting any new patients with Medicare, Medicaid or Tricare.

Cigna (Not in network with Vantage plan)
Humana (Not in network with X plans)
Kaiser Permanente (not in network with Kaiser Select)
Colorado Choice Health Plans
Rocky Mountain Health Plans
Please note this is not a comprehensive list and there may be sub plans of these insurance plans that we are not in network with. Regardless if your insurance plan shows up on our list, please call your specific plan to verify we are in network with your plan. Any balances that come from us not being contracted or in network with your plan are patient responsibility.
Please call our office at 719-266-5244 ext 101 to set up an appointment with one of our providers. Be prepared to leave a message and expect a call back within 24 for 48 hours. We normally do not have the ability for same day new patient appointments.
If you are able, please print up these 4 forms: New Patient Packet PDF , Phone Message Consent, Patient Financial Policy, HIPAA, as well as go over to our expectations/ policy page to become aware of our office policies. If you would like to have your records sent to us before your appointment, please fill out a Records Release and send it to your previous provider.
If you are unable to print up the required paperwork, we need you to show up to our office 30 minutes before your appointment time to fill it out.
Please bring your current insurance card, form of ID and a way to pay your co-pay, which is expected at time of service. We accept cash, check, and every card (Discover, Master Card, Visa, American Express).
