a service to our patients, we bill your insurance company for the
portion of the charges not covered by your co-pay.

Co-Pay |

Your
co-pay is due at the time of your visit. If your co-pay is not paid
in full at the time of your visit, we will add $20 to cover our
administrative costs of billing you unless other billing arrangements
have been made in advance.
